Step 1: Download IRS Form W-9 and save it to your computer. You need Adobe® Acrobat® Reader version 5 or later to download the form. Get Adobe Acrobat.

Step 2: Complete the IRS Form W-9 for your organization by electronically typing in the required fields of the document. Save your changes to the document.
Tip 2. The "Business Name" field should contain the name of the fundraising organization to be paid (for example, "Rolling Hills Elementary School PTA").
Tip 3. The Tax ID Number should be placed in the "Employer identification number" field.
Step 3: Complete all fields of the Enrollment Form.
Step 4: Carefully review and agree to the Schools Program Terms and Conditions and then check the the agreement box.
Step 5: Upload your completed IRS Form W-9 from your computer.
Step 6: Complete the Name, Title and Date fields and click Submit to send your organization's enrollment request. Step 7: Within a few business days, you will be notified via email from Verizon Wireless of your organization’s enrollment status.
